INSURANCE REQUIREMENTS

  • Grace Mount Zion Apostolic Church must be named on the certificate as the legal entity. Our address is 5865B Coopers Ave. Mississauga, ON L4Z 1R9.

  • The certificate must clearly indicate the policy number, start and end date.

  • The insurance must be for a minimum of $2,000,000 in general property and liability per occurrence.
